What is Support Coordination?

Help putting your NDIS plan into action

Support coordination is a funded NDIS support under the Capacity Building budget. It helps participants understand what their plan means, find and connect with the right providers, and build the confidence and skills to manage their own supports over time.

It is not physiotherapy — it is case-management and navigation, billed under its own NDIS item codes, and available to participants who have Support Coordination funding in their plan.

Level 1

Support Connection

Short-term, lighter-touch help for participants with straightforward needs. We help you understand your plan, connect with providers and community services so you can act more independently.

Level 2 — Most Common

Coordination of Supports

Ongoing, active coordination for participants with moderate-to-high complexity across multiple services. We build your ability to design and manage a mix of supports, maintain provider relationships, resolve issues and prepare for plan reviews.

Level 3 — Specialist

Specialist Support Coordination

High-intensity, time-limited case management for participants in complex or high-risk situations — crisis, hospital discharge, justice-system transitions, or multiple intersecting needs. Must be delivered within an expert framework by an appropriately qualified practitioner.

Requires an allied health qualification — which Thrive holds. This is the highest-value, least-contested tier of support coordination.

Who Can Access Thrive SC?

Which Plan Management Types Do We Support?

Plan-Managed

Your plan manager pays providers on your behalf. Thrive can be engaged as your support coordinator — no registration required.

Self-Managed

You pay providers directly. You can engage any provider you choose, including Thrive — no registration required.

NDIA-Managed

Agency-managed plans require a registered NDIS provider. Registration in progress — contact us to discuss your options.

All providers must comply with the NDIS Code of Conduct regardless of registration status. Thrive maintains transparent conflict-of-interest practices and full participant choice across all coordination and therapy services.

Common Questions

Support Coordination FAQ

Does support coordination funding come from my NDIS plan? +

Yes — support coordination is funded under the Capacity Building budget in your NDIS plan, under the support category "Support Coordination." If your plan includes this funding, you can engage a support coordinator without it affecting your Core or other funding.

Can Thrive provide both physiotherapy and support coordination for the same participant? +

Yes, with full transparency. Where the same organisation provides both coordination and therapy, this is a conflict of interest that must be disclosed to the participant and documented. We will always make this clear, provide information about alternative providers, and ensure participant choice is exercised freely. Participant choice and control is our priority.

Do I need to be a registered NDIS provider? +

For plan-managed and self-managed participants, no registration is required. We comply with all NDIS Code of Conduct obligations regardless of registration status. Registration to serve NDIA-managed participants is in progress. If you are NDIA-managed, contact us to discuss your situation.

What's the difference between Level 2 and Level 3? +

Level 2 (Coordination of Supports, $100.14/hr) is the most common level — ongoing, active coordination for participants managing multiple services. Level 3 (Specialist Support Coordination, $190.54/hr) is time-limited, high-intensity case management for complex or high-risk situations. It must be delivered by an appropriately qualified practitioner — typically allied health, social work, or psychology. Not every participant needs Level 3, and not every provider can offer it.

How do I know if I have support coordination in my plan? +

Log in to the myNDIS portal and look under Capacity Building for a support category called "Support Coordination." If you're unsure, submit a referral anyway and we'll help you check what's available in your plan.

Can a GP, hospital or allied health professional refer a participant? +

Yes — we welcome referrals from GPs, hospitals, allied health professionals, case managers, carers and family members, as well as from participants directly. Fill out the referral form above with as much detail as you have and we'll take it from there.
