The Support at Home program (which replaced Home Care Packages in November 2025) is a government-funded program that allows older Australians to access allied health services — including physiotherapy — in their own home.
Thrive's approach uses a Restorative Care Pathway: a goal-based, time-limited physiotherapy program designed to help you regain function, reduce your reliance on assistance and stay at home for longer. We work closely with your care manager and GP to ensure your physiotherapy is coordinated with your broader care plan.
DVA Gold and White Card holders can also access in-home physiotherapy with no out-of-pocket cost through DVA-funded allied health services.
We partner with residential aged care facilities across Parramatta and Western Sydney to provide regular, on-site physiotherapy visits for residents. Our service integrates seamlessly with the facility's care team — no disruption to operations, and no cost to the resident.
Our physiotherapy is funded through the facility's AN-ACC (Australian National Aged Care Classification) government funding, which covers allied health for residents as part of their care. Facility management simply books our service — there is no charge passed on to the resident or their family.
We provide written progress notes to the GP and care team, and work collaboratively with nurses, care workers and family members to deliver a consistent, goal-driven approach to each resident's wellbeing.
We accept referrals from GPs, geriatricians, hospital social workers, care managers and family members. Every referral is acknowledged within one business day, and we send written progress reports back to the referring clinician at regular intervals throughout the program.

Not always. You can contact us directly to book a private assessment without a GP referral. A GP referral is required to access Medicare-funded sessions under a Chronic Disease Management (CDM/EPC) plan. For Support at Home, you'll need to be registered with My Aged Care — your GP can help with this, or you can call 1800 200 422 to get started.
Support at Home is the Australian Government's program that subsidises aged care services for older Australians living in their own home. It replaced Home Care Packages in November 2025. It covers a wide range of services, including allied health like physiotherapy. If you're approved through My Aged Care, physiotherapy sessions can be funded as part of your package with little or no out-of-pocket cost.
For Support at Home clients, the cost of physiotherapy is drawn from your government package — your out-of-pocket cost is typically very low or nil. DVA Gold Card holders access physiotherapy at no cost. Medicare CDM/EPC plans provide a rebate for up to five allied health sessions per calendar year, with a small gap payment. Private and self-funded clients pay directly, and we can provide a fee schedule on request.
Absolutely. We regularly work with family members and carers who are organising care on behalf of an older loved one. You can call us, email us, or use the referral form on this page. We're happy to speak with both the patient and their family at the initial assessment.
We visit the facility at scheduled times to assess and treat residents on-site. Our service is funded through the facility's AN-ACC government allocation, so there is no charge to the resident or their family. Facility managers can contact us to discuss a regular visiting arrangement suited to your resident caseload.
